Advanced Gallbladder Cancer (HER2+++) — A Short Zanidatamab Experience

After three months of hospitalization, my father finally received another anti-cancer treatment.

He has been fighting gallbladder cancer for over two years. Due to poor tolerance to chemotherapy, targeted therapy is the only remaining option. He has already undergone multiple HER2-targeted treatments, including disitamab vedotin, trastuzumab, pertuzumab, pyrotinib, and tucatinib, with limited benefit. This time, we chose to try zanidatamab.

His condition has been difficult. Persistent jaundice, infections, and bile duct obstruction required multiple drainage tubes. Liver function has not fully met treatment criteria, with total bilirubin remaining at 99 μmol/L. Imaging showed rapid disease progression, with the largest liver lesion increasing from 5.7 cm to 8.3 cm within one month. After careful discussion, the medical team agreed to proceed with treatment despite the risks.

We had followed zanidatamab for over a year through DengYueMed. Although the drug was already approved in the U.S., it only became available locally as my father’s condition worsened, a timing that felt painfully late. Seeing the name “Zanidatamab” on the infusion bottle was an emotional moment.

The infusion caused a high fever and chills, which later resolved. By the next day, his strength and appetite gradually returned. That small improvement was enough to remind us why we keep trying.
